Our intentions
At Revive Dental Aesthetics, the greatest compliment our guests can pay us is the introduction of a friend, colleague, or family member. Our Referral Program is designed to express our appreciation for growing our practice family while upholding the highest ethical and regulatory standards of dental healthcare.

Eligibility & Federal Healthcare Exclusion
- Federal & State Healthcare Program Restriction: In compliance with federal and state regulations, including the Federal Anti-Kickback Statute (42 U.S.C. § 1320a-7b(b)) and the Civil Monetary Penalties Law (Beneficiary Inducement Statute), this offer is not valid for individuals enrolled in or covered by any federal or state-sponsored healthcare program, including but not limited to Medicaid, Medicare, Medicare Advantage, TRICARE, or the Children’s Health Insurance Program (CHIP).
- Any patient receiving benefits under these programs is strictly excluded from receiving referral credits, fee reductions, or promotional discounts.
Qualifying Referral Criteria
- A “Referred Patient” must be a new patient who has not previously received clinical care, consultations, or diagnostic evaluations at Revive Dental Aesthetics.
- To activate the referral courtesy, the referred patient must present the referral card or note the referring patient’s name prior to or at the time of their initial comprehensive examination and hygiene visit.
- Referral credits are generated once the new patient completes their initial appointment and necessary diagnostic services.
Nature and Application of Program Courtesy Credits
- In-Office Courtesy Credit: The referral courtesy is an internal, non-cash promotional credit applied directly to the patient ledger toward out-of-pocket dental services or elective aesthetic therapies.
- No Cash Equivalent: Credits have no cash value, are strictly non-transferable, and cannot be redeemed for retail purchases, returned, or exchanged for cash or cash equivalents.
- Private Insurance & Fee-for-Service Rules: For patients utilizing commercial PPO insurance, promotional credits may not be used to waive, discount, or forgive mandatory insurance copayments, deductibles, or coinsurance obligations where prohibited by contractual provider agreements or applicable law. Credits will be applied toward non-covered treatments, elective aesthetic services, or applicable fee-for-service out-of-pocket charges.
- Expiration: Courtesy credits expire 12 months after date of issuance.
General Terms & Practice Discretion
- Non-Combinable: Referral credits cannot be combined with other promotional offers, introductory specials, or membership discounts unless explicitly authorized in writing.
- Program Changes: Revive Dental Aesthetics reserves the right to modify, amend, pause, or terminate the referral program and its terms at any time without prior notice. Void where prohibited or restricted by law.
